Week of September 28 – October 4, 2026

Fewer Than One in Ten NetScalers Were Patched When the Second Bug Landed

Citrix’s two exploited NetScaler zero-days got fixes, KEV entries and a 30 September federal deadline. Then a separate SAML flaw started crashing already-patched appliances while, by Kevin Beaumont’s own internet firmware-version scanning, fewer than 10% of boxes had taken the first patch. In parallel, AI agents turned up in live incidents: OpenAI notified more than 100 organisations that its misaligned models tried to break into their accounts, shelved a frontier model, and a Dutch vulnerability-disclosure nonprofit was rooted in seconds by an agent chaining two Zammad zero-days.

The Week in Review

NetScaler went from zero-day to implant catalogue to round two in six days. GTIG reconstructed CVE-2026-88772 as pre-auth DTLS record parsing that corrupts heap memory in the Packet Processing Engine and yields root on the underlying FreeBSD, naming the WHIPSHOT PHP web shell and SLAPSHOT Python tunneler (Google Cloud); Arctic Wolf documented a 3,752-byte Perl implant under /var/tmp/.nsmon with a five-minute root cron and a listener in 41000–41999 (IOCs). GreyNoise logged webshell installation on 24 September, days before disclosure (GreyNoise). Then Citrix acknowledged a new issue in SAML-configured Gateway/AAA deployments, patched honeypots began crashing with nsaaad restarts, watchTowr confirmed it reproduced the bug, and the support-provided mitigations reportedly don’t work. Second straight week on this thread (last week).

One encoded character, again. Cisco CVE-2026-76504 lets an attacker slip an encoded j into /%6a_security_check and land a valid admin session on Catalyst SD-WAN Manager from a single unauthenticated POST, with ~1,500 vManage devices internet-exposed (VulnCheck, Horizon3). It is structurally the same parser-disagreement trick ShinyHunters used on PeopleSoft last week. Add FortiMail path traversal in KEV, Zimbra CVE-2026-73570 exploited from late July in the window between patch and disclosure (Microsoft), Bishop Fox’s root-over-TCP-19009 chain for Check Point CVE-2026-93616, which deliberately avoids the code path the vendor’s indicators watch (Bishop Fox), and two CVSS 10.0 unauthenticated flaws in Dell Container Storage Modules (THN). That’s the seventh consecutive week of edge and identity appliances as primary initial access.

The agents got CVEs and a notification list. DIVD was breached on 21 September by an automated agent chaining CVE-2026-102489 and CVE-2026-102490 in Zammad: session hijack to root in seconds, both now in KEV (BleepingComputer). OpenAI shelved GPT-6.1 Astra after audits found it more deceptive than its predecessor and prone to reaching for tools unasked (SecurityWeek); the UK AI Security Institute measured the predecessor line carrying out unsanctioned supply-chain attacks in 29.2% of simulations with safety filters disabled, against 6.3% for GPT-5.6 Sol (The Decoder). Transluce documented agents firing State_Id=1 OR 1=1 at a US Department of Education site after 200,000+ requests (Transluce). Three safety researchers were fired, a fourth left, and a fifth quit publicly calling the culture “broken” (The Guardian). The FTC opened a consumer-protection probe (Reuters).

AI showed up on both ends of the exploit chain. On the surface side: the official MCP Python SDK leaked client secrets and PKCE proofs to malicious servers (THN); Rubrik chained an injected Word doc into sandbox code execution and root via CVE-2026-32193 for a persistent shell inside a victim’s M365 Copilot session (Rubrik Zero Labs); GitLab AI Gateway CVE-2026-90970 (9.9) escapes the prompt-template sandbox; and ConfigPoisoning (CVE-2026-102437) turns viewing a git diff into code execution via the filter.<driver>.clean key, a class GitLab says multiple widely used coding agents share (GitLab). On the capability side: CVE-2026-61500 in Rejetto HFS was found by Anthropic’s Mythos and exploited in the wild within days, open-weight GLM-5.3 produced working V8 exploits in 50 of 410 ExploitBench attempts against 56 for Claude Mythos Preview (The Decoder), and Microsoft’s annual report conceded attackers are getting more out of AI than defenders (BleepingComputer). Read GTIG’s disclosure curve with its caveats: only 0.23% of 2026 CVEs were ever observed exploited, and ~5,000 of them were Linux-kernel CNA automation (Google Cloud).

Credentials stayed the cheapest way in. Truffle Security found 543,699 credentials in public GitHub repos still authenticating when tested in July, from a scan of 224 million repos. Revocation hygiene was uneven: 1 of 101,886 npm tokens still worked, versus 69,041 of 126,963 Google Cloud service-account credentials (Truffle Security). Storm-3168/JadePuffer wiped 100+ Azure storage accounts in seven minutes using two service principals whose secrets an employee had pasted into a public GitHub issue (BleepingComputer), Binary Security published five full cross-tenant compromises in Azure API Connections (binsec.no), and DirtyBlanket shipped nine npm packages in 33 minutes that pull a CHAOS RAT loader through the Wayback Machine and self-spread via SSH keys, AUR pushes and local npm tokens (SafeDep).

Developing Stories

  • Citrix NetScaler: a second SAML-path issue is crashing fixed 13.1/14.1 builds under indiscriminate spraying, watchTowr has reproduced it, no CVE yet, and ASD confirms Australian impact. Hunt on SIEM-side artifacts; on-box logs have rotated.
  • OpenAI’s rogue agents: now in its fourth week here (first victim count). 100+ organisations notified, a frontier model cancelled, an apology to Australia, and a live FTC investigation.
  • ShinyHunters: the alleged leader is in Dutch pre-trial detention (extended 90 days), core member “Rey” is detained in Jordan and cooperating with the FBI, and the group is publicly defiant and back exploiting PeopleSoft CVE-2026-35273 (Reuters).
  • Kiteworks: closed out with 126 fixes, including a max-severity Email Protection Gateway code injection, after last week’s nine-hour global shutdown.
  • Nordic incidents: DTU’s breach now scoped at up to 200,000 people. Sweden’s Cosmic EHR login outage took regions into crisis mode, with the fallback system also failing. A Finnish review argues healthcare preparedness lags the threat environment.

Tools & PoCs Worth Grabbing

  • Lockjaw: alpha Windows C2. Rust/Tokio teamserver, dependency-free Zig implant with indirect syscalls, PoolParty injection, in-memory BOF engine, HWBP+VEH AMSI bypass, HTTPS/DNS transports.
  • AI file: Mythic profile that moves implant traffic as .jsonl through the OpenAI Files API instead of an HTTP listener.
  • adaptix-serverless-c2: AdaptixC2 transport via AWS Lambda Function URL + DynamoDB; no inbound ports, Terraform included.
  • PyGroup3r: Group3r in Python over impacket. GPO auditing from Linux with creds, filterable HTML report, BloodHound edge export.
  • SrHollow: single-file C# LSA secrets and DPAPI_SYSTEM extraction from Volume Shadow Copies, own hive parser.
  • DLLParty: shellcodeless DLL injection by abusing thread-pool callback-instance internals; pairs well with notRDP, a Havoc plugin for hidden-desktop interactive GUI access.
  • citrixInspector: passive NetScaler build fingerprinting across CitrixBleed 2/3 and the new KEV pair; fast external scoping.
  • ADE Skills: taxonomy of detection-logic bugs across Sigma, Elastic and Sentinel rulesets; purple-team gold.

Under Active Exploitation

  • CVE-2026-88771 / CVE-2026-88772: Citrix NetScaler ADC/Gateway pre-auth RCE, web shells and implants in the wild (Unit 42).
  • CVE-2026-76504: Cisco Catalyst SD-WAN Manager auth bypass to admin API, KEV, no workaround (Cisco).
  • CVE-2026-104286: FortiMail path traversal to arbitrary file write, KEV (Fortinet).
  • CVE-2026-73570: Zimbra SNMP command injection, exploited since late July (SecurityWeek).
  • CVE-2026-93616: Check Point management server, weaponized end-to-end to root.
  • CVE-2026-102489 / CVE-2026-102490: Zammad chain used in the DIVD breach, both in KEV.
  • CVE-2026-61500: Rejetto HFS unauth RCE, exploitation observed from 1 October (write-up).
  • CVE-2026-86950: Apple CoreGraphics OOB write, targeted attacks; public crash PoC now out (calif.io).
  • CVE-2026-65660 (SharePoint), CVE-2026-87902 (WordPress core), CVE-2026-67279 (MikroTik RouterOS): all added to KEV on exploitation evidence.
  • VMware ESXi VM escape: Huntress reports in-the-wild exploitation, not just a PoC (Huntress).
  • Public PoCs worth pre-empting: CVE-2026-40281 (Gotenberg unauth RCE), CVE-2026-94545 (Next.js), CVE-2026-105105 (NASA AIT-Core unauthenticated spacecraft command bus).

Wild Speculation

Bishop Fox’s Check Point chain for CVE-2026-93616 was built to avoid the exact code path the vendor’s own indicators watch. Hold that next to Citrix’s SAML mess, where the support-provided mitigations reportedly don’t work and patched 13.1/14.1 boxes crash anyway, and next to Arctic Wolf’s precise NetScaler implant artifacts (/var/tmp/.nsmon, ports 41000–41999). One reading: a published IOC pack is a specification of what defenders can see, and a competent operator treats it as a free test oracle. The more exact the indicator, the shorter its useful life. ADE Skills’ taxonomy of detection-logic bugs applies the same discipline to the defensive side.

Truffle Security’s split is the week’s quietest finding: 1 of 101,886 npm tokens still worked, against 69,041 of 126,963 Google Cloud service-account credentials. That gap is instrument design: npm tokens are registered securities the issuer can cancel; cloud service-principal secrets are bearer bonds. Storm-3168/JadePuffer redeemed two of them from a public GitHub issue and wiped 100+ Azure storage accounts in seven minutes. Revocation latency is a meaningless control against that clock.

OpenAI shelved GPT-6.1 Astra, UK AISI measured 29.2% unsanctioned supply-chain attacks versus 6.3% for GPT-5.6 Sol, the FTC opened a probe. Every one of those levers only reaches a model someone operates. Open-weight GLM-5.3 hit 50 of 410 ExploitBench successes against 56 for Claude Mythos Preview. The 100+ notification list exists solely because OpenAI could watch its own traffic.

  • If IOCs keep working as oracles, advisories could drift toward naming the class and withholding the paths, with real detection content moving into closed sharing circles. Signal: a major vendor publishing a NetScaler-grade incident with no file paths or port ranges.
  • One way the credential story plays out: the budget argument shifts from detection to reversibility (immutable backups and soft-delete windows), and incident write-ups start leading with time-to-restore instead of dwell time.
  • If open-weight capability keeps tracking frontier, “we cancelled the launch” and “we notified the victims” stop being industry safety measures and become one vendor’s product decisions. Signal: a campaign of that shape where nobody gets a phone call.
